Because of the nature of the Legacy you wish to leave, it is important that certain details be confirmed that will enable My Hidden Legacy to complete your gift and do it for you at a time of your choosing. We want to deliver your gift with sensitivity and dignity, and will welcome any and all input you may wish to offer that will help us along that path.

First, read the Service Agreement by clicking on the PDF file below.

Second, fill out the Legacy Details portion at the end of the Agreement. Please give us as much information as possible to ensure that your gift is delivered to the proper person(s) at the appropriate time.

Third, you have two options.

• Download the Service Agreement PDF file, sign and date it, then fax it to us at 602-773-1483.

• Or, Download the Service Agreement PDF file, sign and date it, scan it back into your files, and email the document to us.

Fourth, it is critical that the Executor of your estate, or some other person you designate, is informed regarding this issue. You may or may not choose to disclose the details of the gift to this person. However, they will be the one who notifies us upon your passing, which then enables us to begin preparations for the fulfillment of your Legacy. So please, ensure that you have shared this with the appropriate person, and that they know how to notify us in a timely manner.
